2. Rotary Youth Volunteer Information and Declaration Form
In addition to other requirements, Volunteers who could be alone with a young person for a period of time, being such time that might allow grooming or abuse to occur, are required to complete this form and submit this to their CYPO who must then forward this to the DYPO before commencement of the program.
